1. Who is responsible for your data?

Femme Verloskundigen (hereinafter: “we”) is responsible for the processing of your personal data.

2. What data do we process?

Depending on the care and contact, we process, among other things:

• Contact details: name, address, telephone number, email address.
• Identification and insurance details: date of birth, BSN and health insurer (for declaration and file).
• Medical data: data about your pregnancy, health, ultrasounds and treatment (special personal data).
• Data from forms: what you enter when registering, contacting or Keepsake Ultrasound request on this website.
• Website details: anonymized visit statistics (see our cookie statement).

3. For what purpose and on what basis?

• Providing midwifery care — on the basis of the treatment agreement (WGBO) and your consent.
• Scheduling appointments and contacting them in response to a form — based on your request/consent.
• Declaration to your health insurer and administration — on the basis of a legal obligation and the treatment agreement.
• Improving our website — on the basis of our legitimate interest, with privacy-friendly statistics.

4. How long do we keep your data?

We store medical files in accordance with the legal retention period of the WGBO (in principle 20 years, or as much longer as good care provision requires). We do not store data from contact and application forms for longer than is necessary to process your question.

5. Share with others

We only share your data when necessary or mandatory, for example with:

• co-practitioners such as the hospital, the gynecologist, the general practitioner or maternity care (in case of referral or transfer);
• your health insurer (for declaration);
• processors that support us, such as our hosting and form service (Netlify) and our statistics service. Processing agreements have been made with these parties.

We never sell your data to third parties.

This website uses functional cookies and analytical cookies to measure usage. You can read more about this in our cookie statement.

7. Your rights

You have the right to view your data, have it corrected or deleted, and to object to or request restriction of processing. Do you want to take advantage of this? Contact us at info@femme-verloskundigen.nl. We will respond within the legal period.

8. Complaint or question about privacy

Do you have a question or complaint about how we handle your data? Please contact us first. If you cannot reach an agreement with us, you have the right to file a complaint with the Dutch Data Protection Authority. For complaints about the care itself, we refer you to our complaints procedure.
